If you need assistance printing forms or have questions about what forms you need, please give us a call. We know that your time is valuable and want to make things as easy as possible.

Please print two copies of the Litigation Form. Each biological parent or guardian must sign a separate copy for the patient's file.
Please complete ONE of the below forms depending on your insurance preference.
Please complete the Self-Pay Agreement if you are uninsured or do not require submission of claims to your insurance policy.
Please complete the Financial Agreement if you prefer The Talking Place to submit claims to your insurance policy.
